Junior Profile Form

Sussex County Golf Union collect and maintain the Parental Consent Forms. For a complete and up to date list of the Junior Girls whose forms have been completed, click on the link below. By way of a reminder, if your child is playing in a Sussex event or attends a coaching session, a Parental Consent Form MUST be completed and a hard copy must be kept in your daughter’s golf bag when they play.

Any child who arrives to play in an event for whom we do not hold such a form will be unable to play.
